Email account breach at Triad Radiology affects 11,011 people
Triad Radiology Associates said an employee email account may have exposed protected health information and personal data. The HHS breach portal lists the incident as affecting 11,011 individuals.
Triad Radiology Associates has issued notice of a data security incident involving an employee email account that may have contained protected health information.
The Winston-Salem, NC-based radiology practice discovered suspicious activity on or around July 30, 2025, and secured the account before bringing in third-party specialists to investigate, according to the notice.
The investigation found that a limited amount of information may have been accessed between July 11 and September 8, 2025, the practice said on its website.
Following a review completed December 10, 2025, TRA confirmed that personal information may have been accessible to an unauthorized third party. Some information belonged to patients of other medical providers for which TRA provides radiology services, according to the notice.
Potentially affected information varies by individual and may include names, addresses, Social Security numbers, driver’s license numbers, bank account information, dates of birth, medical information, and health insurance information.
TRA posted notice on its website February 6, 2026, and began sending direct notices to affected individuals February 11, 2026. The U.S. Department of Health and Human Services Office for Civil Rights breach portal lists the incident as affecting 11,011 individuals.
The practice said it has no evidence that the information has been viewed or misused. However, it is offering credit monitoring and identity protection services to potentially affected individuals.
TRA also said it has reviewed its data security policies and procedures and implemented additional security measures.
Individuals with questions can call 1-800-405-6108 Monday through Friday from 8 AM to 8 PM ET, excluding holidays. The practice said it has notified HHS OCR and applicable state regulators.
